Merge branch 'rs/daemon-interpolate' into maint

"git daemon" looked up the hostname even when "%CH" and "%IP"
interpolations are not requested, which was unnecessary.

* rs/daemon-interpolate:
  daemon: use callback to build interpolated path
  daemon: look up client-supplied hostname lazily
