Documentation: make it easier to maintain enumerated documents

Browse Source 
Instead of starting an enumeration of documents with a DOC = doc1
followed by DOC += doc2, DOC += doc3, ..., empty it with "DOC =" at
the beginning and consistently add them with "DOC += ...".
