revisions.txt: remove ambibuity between <rev>:<path> and :<path>
The revision ':README' is mentioned as an example for '<rev>:<path>' but the explanation forwards to the ':<n>:<path>' syntax. At the same time ':<n>:<path>' did not mark the '<n>:' as optional.
