parseopt: add PARSE_OPT_NO_INTERNAL_HELP

Add a parseopt flag, PARSE_OPT_NO_INTERNAL_HELP, that turns off internal
handling of -h, --help and --help-all.  This allows the implementation
of custom help option handlers or incremental parsers.
